First appointment: What do you wear?

Ideally, you should come wearing athletic and comfortable clothing that allows us to see the painful area.

Does your Health Insurance cover the costs? 

We will provide an insurance form for each session (if requested).  You can file the form with your insurance company.  If you have a PPO, they will likely cover a portion of your fees.

Scheduling

Sessions are scheduled by appointment only. We make every effort to accommodate your busy schedule and can usually get you started right away!

Thefollowing patient registration forms are in an easy to open packet that can be downloaded directly from our website. You can print and complete these documents in advance if directed to do so and bring them to your first appointment. If you have any questions about these forms or you would like to fill them out at our office, please arrive 10-15 minutes early on the day of your appointment to allow time to complete your forms.
